A brief overview of the Pulmonology Specialized Center

In our specialized center we provide prevention, diagnosis, treatment, assessment and research on disorders of the respiratory tract including tuberculosis. The members of our team also perform lung transplants. In addition, we focus on reducing the probability of transition from acute to chronic stage, along with rationalization of care for patients with chronic diseases of the respiratory tract.

In 2014, 1.714 patients were hospitalized. We treated 1.598 outpatients, performed 1.485 bronchoscopies and provided 1.126 patients with chemotherapy.

Is this placement ideal for you?

  • Despite though you will observe many diagnostic and examination methods and intensive work with the patients, the pulmonology specialty is also a surgical one. Within the morning doctor´s rounds you will visit the center's ICU where are hospitalized our patients in severe conditions. You can see the specialty is truly wide, one day it will teach you how to work with laboratory or x-ray results, other day you meet many patients and next day you might be standing in the operating room. The specialty is the best one for you, if you wish to experience as many aspects of doctor's work as possible.
  • You will appreciate pulmonology mainly once considering your career as a future doctor, the job tasks are connected to the medical specialty. We cooperate with many other departments in the hospital, our patients prior or after lung transplant do the physical therapy under the supervision of experienced professionals. We have a special sleep laboratory and cooperate with chest surgeons and oncologists mainly.

The Pulmunology Specialized Center is concerned with diagnostics, treatment, evaluation and research of diseases of the lower respiratory tract with emphasis onto intensive pneumological care, interventional bronchology, comprehensive care for patients with COPD (Chronic Obstructive Pulmonary Disease), treatment of patients with cystic fibrosis, oncology including biological therapy, sleep medicine, examination of patients prior to lung transplantation and also, the treatment of tobacco addiction. In 2014, we treated more than 3.000 patients.

In our department we use the best equipment, such as the ultrasound bronchovideoscope and the department's ICU is also equipped with a modern non-invasive ventilator that can be used during the transport of patients.

Our doctors have contributed to the establishment of a standard protocol for lung transplant procedure and attend international events such as the European Cystic Fibrosis Conference. Our hospital was the first one to treat cystic fibrosis patients with the cystic fibrosis transmembrane conductance regulator (CFTR),  Kalydeco.

Our center consists of the following specialized outpatient facilities:

  • Diseases with bronchial obstruction

  • Interstitial pulmonary disease

  • Transplantations
  • Treatment of tobacco addiction
  • Patients with sleep breathing disorders

Who do we take care of?

In addition to pulmonary treatment and diagnostic procedures such as biopsy of lungs, mediastinoscopy, and we also deal with lung carcinoma (resection surgery, radiotherapy, chemotherapy, biological treatment, and immunotherapy). In very complicated cases that require multidisciplinary involvement we consult with specialists from other departments.

In addition to tuberculosis, we treat patients who suffer from diseases such as bronchial asthma, chronic obstructive pulmonary disease, cystic fibrosis, oncology diseases, pneumonia, inflammatory or malignant pleural effusion, incl. empyema, critical stenosis of the trachea and bronchi, pneumothorax, a disease with massive hemoptysis, pulmonary fibrosis in respiratory failure and diagnostic procedures - In addition to regular fibrobronchoscopy with biopsy sampling, we carry out bronchoalveolar lavage, peribronchial biopsy of lymph nodes and bronchoscopic lung biopsy.

We also perform spirometry, body plethysmography, bronchodilator tests and examination of diffusion, as well as an examination of patients prior to their lung transplantation and patients with sleep apnea syndrome.

Our methods and procedures

Our coordinated team performs these procedures within the department:

  • Biological treatment of lung cancer patients with ALK-positive breakthrough using the crizotinib medicament;
  • Endobronchial ultrasonography (EBUS) allowing puncture mediastinal lymph nodes under ultrasonographic control in real time with immediate cytological evaluation findings using ROSE (Rapid On Site Evaluation).
  • The introduction of bronchoscopic lung cryobiopsy, which is a method that, in some cases ,obviates the need for  open lung biopsy

How does the usual schedule look like?

  • The department is mostly busy during the day shift as the procedures are typically planned. However, you will get familiar with the schedule of your placement week on Monday morning, 25 hours as a minimum are guaranteed weekly.

Our department is the top extra specialized workplace at a nationwide level. I am very happy that my serious interest in Pulmonology during university studies became a reality here in the University Hospital in Motol. Although my childhood dream was to become a doctor, as the six previous generations of my family also chose to do, I didn’t expect the challenges on the path of pursuing it.

Why did I become a healthcare professional?

During high school, I started to prepare for admission to a medical university. At the time, being accepted was the most important thing that had happened to me in my life. The goal that I strived for, step by step from my childhood was fulfilled, and I graduated with honors. My professional hero was my dad, who was working in a center of pulmonology. I wanted to be like him. I worked at a Medical Faculty in Russia at the I. P. Pavlov Medical University. But sometimes life brings completely unexpected challenges. I left my home country and moved to the Czech Republic. After overcoming obstacles, I passed the medical exams and I was able to get back into my profession.

Actually, ever since my childhood I felt determined that one day I would also wear a white lab coat. It is very important in life to do what you love and to be completely dedicated to it.

What do I love the most about my specialty?

I like to be a part of a highly professional and coordinated team of qualified world class specialists in pulmonology. I appreciate the opportunities for cooperation in the international scene in our hospital and I'm glad that in our department I do not perform only routine procedures, but also scientific and research activities.

What can you see by shadowing our team?

You will see the everyday medical tasks in the Pulmonology center - physical examinations of patients and assessing the results of laboratory tests and imaging tests. I perform bronchoscopies, as well as diagnostic and therapeutic procedures like thoracentesis, SONO of the chest, drain extraction, blood administration, and so on. Once a week, there are important patient rounds with the head of our specialized center. We analyze medical records of patients and plan the following steps for treatment - the diagnostic procedure. Once a week you can also see the interdisciplinary seminar. There is always a chest surgeon, an oncologist, and other specialists. We discuss bronchoscopies, chest punctures or work in the acute outpatient pulmonology center. I take care of newly admitted patients, discharging patients, and the transport of patients to other hospitals or departments.

I like to spend time with my family. I love travelling and seeing the world.

Pulmonology is an exciting field that is evolving all the time. You need to have a knowledge of internal medicine, radiology, oncology, microbiology, urgent care medicine, and cardiology , as well as manual skills for working with an endoscope. I always perform my work with humility, as I appreciate that I can help sometimes even severely injured patients whose lives are somewhere between life and death.

Why did I become a healthcare professional?

I come from a family of doctors, and since my childhood I have been interested in natural sciences, especially biology. During high school I was fascinated by chemistry and physics. I decided well ahead of time to apply for medicine. As I studied, I was motivated not only by a desire to help people, but also by a desire to understand anatomy and the complex systems of the body. While studying I had the opportunity to take part in internships abroad; I visited France, Montenegro, and Finland. These experiences improved me from a professional point of view, as well as in my personal life.

I always perform my work with humility, as I appreciate that I can help sometimes even severely ill patients suffering from cystic fibrosis or lung cancer whose lives are somewhere between life and death.

What do I love the most about my specialty?

Even though my work is sometimes routine (mostly the grueling administrative tasks), I still participate in new and interesting clinical studies, learn new things, and gain valuable experiences and practical skills. There are new diagnostic procedures that make their way into daily practice more and more. Great developments can be seen mainly by the most ill patients suffering from lung cancer or cystic fibrosis.

What can you see by shadowing our team?

During the day you will participate in the doctor’s rounds (including in the ICU), the physical examination of patients, and see lab and x-ray results. Another experience you may gain is seeing a bronchoscopy examination, including complex diagnostic and therapeutic procedures that are conducted under total anesthesia. Further you may see spirometry, cytology, or a chest ultrasound examination, including the conduction of a diagnostic or evaluation of a thoracentesis (pleural puncture). You may meet patients before and after lung transplantations, as well as cystic fibrosis patients.

Apart from being a healthcare professional...

I would describe myself as a sports fan. I like fitness, swimming, and biking. If I am too tired to play sports, I enjoy reading.
